What is the reason that’s responsible for failure of waterfall model?

Most of the SDLC models are somehow derived from traditional waterfall
models. There are some well-defined reasons that caused the failure of waterfall  model.  

a) Its one-way lane approach: Here you can move to the next phase only
when you are done with the previous phase.  

b) It’s no going back approach: If Phase X is completed and you move to
Phase Y, no changes can be made in Phase X now.  

c) Lack of interaction: There was little, or no interaction involved with the teams
operating as well as between the product owner.
